Provider ID: For most users this will be the Medicare Provider Number of the user's facility. This is a six digit number with the form "NN-NNNN". Some corporate users will use a corporate name instead of a particular facility's provider number.

Username: This is the user's designated user name. This account is created by the user's facility's PACCtrax adminstrator.

Password: This is the user's designated password. For security reasons it must be at least eight characters and be a mix of alpha and numeric characters. The facility's PACCtrax administrator establishes and can modify these passwords.
